An exciting opportunity has arisen for an experienced, enthusiastic and highly motivated Clinical Pharmacist to join our new virtual ward service to support direct patient care and be part of our newly forming team within CCICP community services.

The Virtual Ward will care for patients with an acute medical episode, in their own homes who require further acute health care, monitoring and treatment from the multidisciplinary team to prevent avoidable admission to hospital, or to support early discharge out of hospital.
